Direct Deposit Authorization Form for Contractors

Contractors will be paid each Tuesday of the month based on the number of hours worked, and agreed upon hourly rate set forth in the independent Contractor and Agency Agreement Form. If a holiday should occur on a payday, contractor will be paid the business dey prior.

Seniorly Yours, LLC is hereby authorized to direcitly deposit my pay into the above stated account electronically. This authorization wili remain in effect until the above account holder notifies Seniorly Yours in writing or gives notice of termination of contracted service
